I recently struggled with issues surrounding patches and automatically deployed agents.  When hotfix 954049, 958254, and 957511 was installed, the Ops Mgr installation did not indicate that any agents needed an udpate.  I used Kevin Holman's blog entry to verify that the right patches were installed and followed the instructions in each KB article to verify that the right files were updated on the management servers. 
 
I finally made a call to support and found out the following information.
 
When you install an Ops Mgr hotfix on a management server you *must* install the hotfix using the SetupUpdateOM.exe file that is located with the hotfix.  It is also recommended that you run the executable from a command prompt (CMD.EXE) with administrator permissions. 
When the process of updating the management server nears completion, the SetupUpdateOM.exe program will copy the proper MSP file into the right agent directory.  For example:
 
The x64 MSP file will copy to the following location:
C:\Progrma Files\System Center Operations Manager\AgentManagement/AMD64
 
The x86 MSP file will copy to teh following location:
C:\Progrma Files\System Center Operations Manager\AgentManagement/x86
 
If you run the MSP file manually, this process will not happen and you will not recieve any pending actions to update agents with the recently installed hotfix.
After running through this process on the hotfixes listed above, all hotfixes that should be deployed to agents appeared in the pending actions view.
 
The only issue I ran into after that was the improper agent version reporting to the Ops Mgr console.  When I figure this out, I will post an update.
